

Service Coordinator responsibilities
In addition to coordinating the early intervention services listed in your Individualized Family Support Plan (IFSP) and meeting regularly to discuss your child’s progress, your service coordinator will:
- Conduct first contact activities
- Discuss current concerns
- Ensure procedural safeguards
- Contact your family at least every three months by telephone or face-to-face
- Discuss your family’s satisfaction with the support and services you’re receiving
- Provide information that will help your family with your child’s development
- Provide information about parent and advocacy support organizations
- Assist with transition activities when exiting the program
